Welcome to Gordon Physical Therapy! As a patient, it is important that you feel comfortable with the care you are receiving and that you know exactly what to expect from us.

If you’re coming to us for Physical Therapy, please fill out the “New Patient Forms” prior to your initial appointment (this will save you some time in the clinic).

Lastly, you can view our “Notice of Privacy Practices” if you’d like. However, you don’t need to print these forms out, unless you’d like them for your own records.

If you have these filled out before your arrival, please arrive 15 minutes before your appointment so that we can get them processed. If you need to complete the forms in our office, we ask that you arrive 30 minutes early to ensure adequate time.

Be sure to bring your referral paperwork (if required), as well as photo identification and insurance information.

Once your appointment has begun, you will have a thorough examination by one of our therapists in order to determine precisely which services you need. This exam will take approximately 40 minutes, and you will need to be wearing comfortable clothing so that you can move easily. Your total treatment time may last up to 60 minutes, if you and your therapist decide that heat or ice will be beneficial. At the completion of your evaluation, your therapist will inform you of the results of your evaluation, including the nature of your pain or injury. You and your therapist will also work together to develop your therapy goals and treatment plan.

As a returning patient, we ask that you arrive at the clinic 15 minutes before your appointment time to update your paperwork. Please let us know if you have changed your address or phone number, or if you have additional insurance information to present.

Be sure to bring your referral paperwork (if required), as well as photo identification and insurance information.
